Patient is a fifty eight year old male presenting with intermittent chest pain radiating to the left arm, onset approximately two weeks ago. History of hypertension and type two diabetes, currently on metformin and lisinopril. Blood pressure today one forty two over eighty eight, heart rate seventy six. ECG shows nonspecific ST changes, troponin negative. Plan is to schedule a stress echocardiogram, start atorvastatin forty milligrams daily, and follow up in two weeks. Patient counseled on diet modification and advised to present to the emergency department if pain becomes severe or persistent.
